R.ewarding I.ndividual S.acrifice and E.xcellence (R.I.S.E.)

Aloha Nui Family Practice, LLC is proud to offer the R.I.S.E. Scholars Award.
The R.I.S.E. Scholars Award is a scholarship given to a graduating island of Hawaii high school senior who has been accepted to college within the Big Island of Hawaii. The R.I.S.E Scholars Award was created to reward an individual's sacrifice and excellence to improving the health and well-being of our hawai'ian island community.

Awards are based on a individuals merit and commitment to serving our island community.

1. Please complete the necessary information below.
2. Include an acceptance letter from a local community college or UH-Hilo.
3. Submit a letter of recommendation from a high school teacher, counselor or principal.
4. A required essay titled "Working For Change". In 500 words or less describe yourself, your understanding of the importance of community service, give examples of your commitment to improving our community, and how this has impacted and influenced your future career.

One student is selected yearly as a R.I.S.E. Scholar Award recipient.
A monetary award of $500.00 is awarded each spring semester in May.

Deadline for all materials to be received is May 1st. Awards will be given June 1st.
